P & P Transitions — Personal & Professional Change Navigator

A free interactive tool for understanding personal and professional transitions, identifying your current phase, and finding practical next steps for navigating change.

Overview

Change happens around us. Transition happens within us.

P & P Transitions is a free interactive resource designed to help individuals and organizations better understand and navigate the human side of change.

Using an integrated four-phase change and transition model, the app helps users explore where they are in the transition process, recognize the challenges associated with each phase, and identify practical steps toward moving forward.

The model includes four interconnected phases:

  1. Acknowledge Losses — Recognize and honor what is ending.
  2. Navigate Uncertainty — Work through the questions, emotions, and challenges of the transition period.
  3. Build New Skills — Develop the capabilities and confidence needed for a changing reality.
  4. Move Forward Together — Strengthen commitment, establish new practices, and recognize progress.

An additional Monitor & Maintain component encourages ongoing reflection, learning, and adjustment.

The app includes separate Personal and Professional Phase Indicators, information about each phase, and personalized recommendations based on the user’s responses. Users can also obtain a downloadable PDF report.

Whether you’re navigating a personal life change, supporting employees through organizational change, or guiding a team through uncertainty, P & P Transitions offers a practical starting point for understanding the transition process.

How to use the P&P Transitions App

  1. Launch the app. Select the Launch App button to open the P & P Transitions Resource Center.
  2. Choose your context. Select the Professional Phase Indicator for workplace or organizational changes, or the Personal Phase Indicator for life transitions.
  3. Complete the indicator. Respond to the questions based on your current experience, rather than how you think you should be responding.
  4. Review your results. Consider the phase identified and the personalized recommendations. Treat the results as a guide for reflection and conversation, not a fixed label or diagnosis.
  5. Explore the transition model. Read about the four phases and the ongoing Monitor & Maintain component. Pay particular attention to the challenges and opportunities that connect with your circumstances.
  6. Identify your next best step. Choose one or two practical actions that will help you move forward. Consider what you need to acknowledge, understand, learn, or do differently.
  7. Save and revisit. Download your PDF report if desired. Return to the tool periodically to reflect on your progress and changing needs.
